Select Defects > Show Defects from the drop-down menu.
Select the release, iteration, or build for which you want to report a defect using the toolbar navigation drop-down lists, if necessary.
Then select Add New.
Enter the defect title and status in the Create Defect box that appears.
Defect logging is a method of identifying flaws in an application or product by testing or recording customer feedback and then creating new versions of the product that address the flaws or the feedback.
Give the bug a number. Each flaw should be assigned a unique ID number.
Choose the appropriate title.
Be precise.
Make it easy to read.
Create a list of reproducible steps.
Include the intended outcomes.
Document everything.
Don't use a demanding tone.
